1-List your name, address, city, state and zip code along  with a phone number that is best to use.

2- List bait type and size, the color you want, total packages you want of that bait, cost per bait and total cost.

Do this for each item!!!!! 

3- add up your total  (wis residents add 5.5% sales tax) and add shipping charges. (refer to shipping chart at bottom of page)

4- you can hand write if you like but printing on the computer is much easier to use.
